Forms

Students should contact the Melbourne Graduate School of Management Student Centre with all queries regarding administrative matters. The following forms should be submitted in person or by mail to:

Advanced Standing

Applications for Advanced Standing (credit and exemption) must be submitted with the subject outline for subjects taken from the Handbook from the year in which the subject was completed. Students should refer to the Melbourne Graduate School of Management Advanced Standing Policy for further details.

 

Assignment Cover Sheets

Prior to completing the Assignment Cover Sheet, students should first check the Learning Management System (LMS) to see if a pre-filled form has been made available. The forms below should only be used if there is no cover sheet accessible through the LMS.

 

Assignment Extension Request

Students with a valid reason for an assignment extension must complete this form and submit supporting documentation to the Melbourne Graduate School of Management Student Centre no earlier than two weeks prior to the assignment due date.

 

External Student Subject Enrolment

This form is to be completed by students not enrolled in a Melbourne Graduate School of Management program, who are requesting permission to enrol in subjects offered by the Melbourne Graduate School of Management with the purpose of being credited towards a graduate degree in another Faculty. Please note that this form needs to be completed with all relevant signatures from the Melbourne Graduate School of Management before submitting to your home Faculty for enrolment.

 

Cross-Institutional Study (Outgoing)

This form is to be completed by local students (Australian citizens, Permanent Residents and holders of Humanitarian Visas, and New Zealand citizens) currently enrolled in a Melbourne Graduate School of Management program at The University of Melbourne who are requesting permission to enrol in a subject or subjects offered by another institution, for the purpose of credit toward their University of Melbourne degree. This is known as Cross-Institutional Study.

 

Internal Transfer

Applications for Internal Transfer for continuing students are open until three weeks before semester begins. After this date, students need to see their Academic Program Manager to discuss their request to transfer programs. New students commencing their first semester of study may not be eligible for Internal Transfer until after completing their first semester of study. In this instance, it is advised that new students seek study planning advice from their Academic Program Manager prior to enrolling, to ensure their first semester subjects will be credited to their preferred program.

 

Leave of Absence and Discontinuation

Students wishing to cancel their enrolment in, or 'discontinue' their program, must notify the Manager (Melbourne Graduate School of Management) in writing by completing the

Local students wishing to take Leave of Absence should use this form also.

International students wishing to take Leave of Absence should read the Information Sheet (PDF, 98kb) and use the



Subject Changes

Students should refer to the Online Handbook for program structure and subject details. Current enrolment details may be viewed via your Student Portal. Students may amend their enrolment via the Student Portal or by submitting a Subject Change form. All subject changes must be approved by the Melbourne Graduate School of Management
